
Complete and sustained abstinence from alcohol can result in the elimination of complications, even in severe instances of alcoholic liver cirrhosis. This conclusion was reached by researchers from the Medical University of Vienna. Their work has been published in the Journal of Hepatology (JH).
Cirrhosis is a condition where the liver progressively becomes covered in scar tissue, leading to a loss of its functional capacity. In Western nations, prolonged alcohol abuse is a primary driver of cirrhosis. This disease frequently brings with it serious complications, such as fluid accumulation in the abdomen (ascites), cognitive impairment (hepatic encephalopathy), and bleeding from enlarged esophageal veins.
It was previously assumed that the emergence of these complications signaled an irreversible stage of the illness. Nevertheless, this recent study demonstrates that liver condition can see improvement even under these circumstances.
The study involved 633 individuals diagnosed with alcoholic cirrhosis, recruited from 17 specialized centers across Europe and Asia. All participants commenced total abstinence from alcohol following the onset of their disease complications.
The follow-up period extended up to five years. During this time, approximately one-third of the patients experienced a complete disappearance of their cirrhosis complications, accompanied by an improvement in liver function. Researchers have termed this state “decompensation” or “disease reversal.”
“Our findings indicate that even subsequent to the appearance of serious complications, cirrhosis is not necessarily an irreversible progression,” stated Benedikt Hofer, the study’s lead author.
According to the scientists, the critical factor enabling recovery was the complete and early cessation of alcohol intake. The sooner patients achieved sobriety, the greater the likelihood of their liver function improving.
The research also revealed a substantial impact of this recovery on patient survival. Among the group whose liver function normalized and who maintained alcohol abstinence, there were no fatalities attributed to liver disease. Furthermore, they had a significantly reduced incidence of developing liver cancer.
“Recompensation is no longer merely a theoretical notion; it represents a clinically significant state capable of substantially enhancing the prognosis, even in cases of advanced cirrhosis,” emphasized Thomas Reiberger, the principal investigator.
The paper’s authors stress that total abstinence from alcohol remains the most potent treatment strategy for alcoholic liver disease. Crucially, sustaining this effort requires robust patient support—including addiction assistance programs and mechanisms to maintain prolonged sobriety. A relapse into alcohol use dramatically worsens the disease outlook.
