
European researchers have determined that the medication known as sultiam holds promise for alleviating the symptoms associated with obstructive sleep apnea (OSA), a prevalent breathing disorder during slumber. A recent clinical trial demonstrated that this drug substantially reduced the frequency of nocturnal breathing cessations and boosted blood oxygen saturation levels. This work has been detailed in the journal The Lancet.
Obstructive sleep apnea manifests when the upper airway repeatedly collapses while an individual is asleep. This results in brief interruptions to breathing, subsequent drops in oxygen levels, and constant awakenings. If left untreated, this condition escalates the risk of developing hypertension, cardiovascular diseases, stroke, and type 2 diabetes.
Currently, the primary treatment modality involves employing a specialized mask that delivers pressurized air to maintain an open airway. While this method is recognized as effective, a considerable number of patients struggle with prolonged adherence, with up to half discontinuing therapy within a year due to discomfort.
The investigation enrolled 298 individuals diagnosed with moderate to severe sleep apnea. Participants were randomly assigned to various groups: a quarter received a placebo, while the remainder were administered different dosages of sultiam. The experiment was conducted across four European nations under a double-blind protocol, meaning neither the participants nor the research staff knew who was receiving the active drug.
The outcome data indicated that patients treated with higher concentrations of sultiam experienced an approximate 47% reduction in the number of sleep-disordered breathing events compared to the placebo cohort. Furthermore, their overnight blood oxygen levels improved. Most of the adverse effects documented were categorized as mild and transient.
According to the scientists, sultiam exerts its influence by modulating the body’s respiratory control mechanisms. It enhances the physiological drive to breathe, thereby mitigating the likelihood of the upper airway collapsing during sleep—the very event that typically triggers obstructive apnea.
“We have pursued this treatment strategy for a considerable period, and these findings confirm that sleep apnea can indeed be targeted pharmacologically. This represents a significant breakthrough,” commented Jan Hedner, a senior professor of pulmonology at the Sahlgrenska Academy, University of Gothenburg.
He further specified that the research team’s subsequent objective is to undertake larger and more extensive studies to ascertain if the beneficial effects persist over the long term and to evaluate the overall safety profile of this therapeutic approach across a broader patient base.
