At the 2026 ASCO Annual Meeting, a recurring theme emerged: the field is moving beyond proving that geriatric assessment works and toward determining how to deliver geriatric oncology care efficiently and consistently in everyday practice. Landmark randomized trials (GAP70+, GAIN, INTEGERATE, GERICO) have demonstrated that geriatric assessment-guided interventions can reduce treatment toxicity, improve communication, and support treatment decision-making for older adults with cancer. As a result, ASCO, NCCN, and SIOG have recommended routine geriatric assessment for older adults receiving systemic therapy. The focus now is on scalable implementation strategies, including remote assessment models, to ensure these evidence-based practices reach patients worldwide.
